1) YOU WILL BE TURNED AWAY if "Permission to Treat" Form is NOT signed by a parent/guardian.

2) ALL 2024-25 athletes are REQUIRED to have a current physical on file in the MHS office in order to participate in sports!

3) IHSA recognizes a physical to be valid for 395 days (athletes are ineligible when current physical expires).

4) All IHSA and MHS forms REQUIRE a Parent Signature (see below).

5) MHS will offer FREE Physicals on May 13, 2024 at MHS gymnasium, 2:30-5pmpm. You are on your own after this date!!!!!

6) Incoming Frosh athletes are required to have a full physical at a Doctor's office and must be turned in during registration.
